Device Story

The NCB Periprosthetic Femur Polyaxial Locking Plate System is a bone fixation device used for stabilizing long bone fractures and osteotomies. This submission introduces new optional provisional instruments to the existing system. These instruments are surgically invasive, non-sterile, and reusable; they are used intra-operatively by surgeons to determine appropriate implant length during planning. The system utilizes metallic bone fixation appliances. The provisional instruments are constructed from polyphenylsulfone with barium sulfate. The device is used in an operating room setting. The output of the provisional instruments is physical guidance for implant selection, which assists the surgeon in the implantation process, potentially improving surgical accuracy and patient outcomes.

Clinical Evidence

No clinical data. Bench testing only. Bending tests assessed handling force behavior. Material testing (polyphenylsulfone with barium sulfate) compared radiolucency and physico-chemical stability pre- and post-sterilization, showing no significant degradation. Cytotoxicity testing confirmed no leachable substances in cytotoxic concentrations.

Indications for Use

Single/multiple component metallic bone fixation appliances and accessories are devices intended to be implanted consisting of one or more metallic components and their metallic fasteners. The devices contain a plate, a nail/plate combination, or a blade/plate combination that are made of alloys, such as cobalt-chromium-molybdenum, stainless steel, and titanium, that are intended to be held in position with fasteners, such as screws and nails, or bolts, nuts, and washers. These devices are used for fixation of fractures of the proximal or distal end of long bones, such as intracapsular, intertrochanteric, intercervical, supracondylar, or condylar fractures of the femur; for fusion of a joint; or for surgical procedures that involve cutting a bone. The devices may be implanted or attached through the skin so that a pulling force (traction) may be applied to the skeletal system.

Malunions {1}------------------------------------------------ KII2174(2/2 Page 2 July 26, 2011 #### Comparison to Predicate Device: Performance Data (Nonclinical and/or Clinical): The modified NCB Periprosthetic Femur Polyaxial Locking Plate System contains the same implants and instruments as the original NCB Periprosthetic Femur Polyaxial Locking Plate System, except for the addition of new optional provisional instruments. Non-Clinical Performance and Conclusions: A bending test was performed to assess the behavior of the Provisionals under high handling force. The acceptance criteria were successfully met. Radiolucency and the physico-chemical stability of the polyphenylsulfone with barium sulfate material was compared before and after cleaning and sterilization cycles - results showed no discernable or significant differences between pre- and postprocessing. There was no evidence of surface changes, etch degradation, material deformation, significant dimensional changes and/or cracking of the provisional materials. The acceptance criteria were successfully met. Cytotoxicity of the polyphenylsulfone material with barium sulfate was evaluated post-processing. No leachable substances were released in cytotoxic concentrations from the test item. The acceptance criteria were met. In addition, this material has been used in prior orthopedic instrumentation with the same ISO 10993-1 classification and has a long successful history of clinical use.
