Indications for Use

To determine bacterial susceptibility to Levofloxacin

Device Story

MicroScan® MICroSTREP plus™ Panel is a miniaturized broth dilution susceptibility test for aerobic non-enterococcal streptococci. Panels contain dehydrated antimicrobial agents; rehydrated with Mueller-Hinton broth supplemented with 2-5% lysed horse blood after inoculation with standardized organism suspension. Following 20-24 hour non-CO2 incubation, MIC is determined by manual visual observation of the lowest antimicrobial concentration inhibiting growth. Used in clinical laboratories to guide antimicrobial therapy by identifying bacterial susceptibility to Levofloxacin (0.002 - 16 mcg/ml).

Clinical Evidence

Bench testing only. Performance evaluated using fresh and stock efficacy isolates and stock challenge strains. Compared against NCCLS frozen Reference Panel. Results showed 99% Essential Agreement for Levofloxacin. Reproducibility and Quality Control testing demonstrated acceptable results.

Technological Characteristics

Miniaturized broth dilution susceptibility test. Dehydrated antimicrobial agents in panel wells. Rehydrated with Mueller-Hinton broth with 2-5% lysed horse blood. Manual visual readout of MIC. Non-CO2 incubation for 20-24 hours.

Indications for Use

Indicated for determining antimicrobic susceptibility of aerobic non-enterococcal streptococci, including Streptococcus pneumoniae.

Regulatory Classification

Identification

An antimicrobial susceptibility test powder is a device that consists of an antimicrobial drug powder packaged in vials in specified amounts and intended for use in clinical laboratories for determining in vitro susceptibility of bacterial pathogens to these therapeutic agents. Test results are used to determine the antimicrobial agent of choice in the treatment of bacterial diseases.

The antimicrobial susceptibility tests are miniaturizations of the broth dilution susceptibility test that have been diluted in water and dehydrated. Various antimicrobial agents are diluted in water, buffer or minute concentrations of broth to concentrations bridging the range of clinical interest. Panels are rehydrated with 115 ul Mueller-Hinton broth supplemented with 2-5% lysed horse blood (LHB), after inoculation of the broth with a standardized suspension of the organism. After incubation in a non-CO2 incubator for 20-24 hours, the minimum inhibitory concentration (MIC) for the test organism is manually read by observing the lowest antimicrobial concentration showing inhibition of growth. The proposed MicroScan® MICroSTREP plus™ Panel demonstrated substantially equivalent performance with streptococcal isolates when compared with an NCCLS frozen Reference Panel, as defined in the FDA DRAFT document "Review Criteria for Assessment of Antimicrobial Susceptibility Devices" (dated May 31, 1991). The Premarket Notification (510[k]) presents data in support of the new MicroScan® MICroSTREP plus™ Panel with Levofloxacin. The external evaluation was conducted with fresh and stock Efficacy isolates and stock Challenge strains. The external evaluations were designed to confirm the acceptability of the proposed MICroSTREP plus™ Panel by comparing its performance with an NCCLS frozen Reference panel. The MICroSTREP plus™ Panel demonstrated acceptable performance with an overall Essential Agreement of 99% for Levofloxacin when compared with the frozen Reference panel. Reproducibility testing demonstrated acceptable reproducibility with Levofloxacin. Quality Control testing demonstrated acceptable results for Levofloxacin. {1}------------------------------------------------ Image /page/1/Picture/1 description: The image shows the logo for the U.S. Department of Health and Human Services. The general controls provisions of the Act include requirements for annual registration, listing of devices, good manufacturing practice, labeling, and prohibitions against misbranding and adulteration. If your device is classified (see above) into either class II (Special Controls) or class III (Premarket Approval), it may be subject to such additional controls. Existing major regulations affecting your device can be found in the Code of Federal Regulations, Title 21, Parts 800 to 895. A substantially equivalent determination assumes compliance with the Current Good Manufacturing Practice requirements, as set forth in the Quality System Regulation (QS) for Medical Devices: General regulation (21 CFR Part 820) and that, through periodic QS inspections, the Food and Drug Administration (FDA) will verify such assumptions. Failure to comply with the GMP regulation may result in regulatory action. In addition, FDA may publish further announcements concerning your device in the Federal Register. Please note: this response to your premarket notification submission does not affect any obligation you might have under sections 531 through 542 of the Act for devices under the Electronic Product Radiation Control provisions, or other Federal laws or regulations. {2}------------------------------------------------ Page 2 This letter will allow you to begin marketing your device as described in your 510(k) premarket notification. The FDA finding of substantial equivalence of your device to a legally marketed predicate device results in a classification for your device and thus, permits your device to proceed to the market. If you desire specific advice for your device on our labeling regulation (21 CFR Part 801 and additionally 809.10 for in vitro diagnostic devices), please contact the Office of Compliance at (301) 594-4588.
