Indications for Use

FRAXION SPINAL FIXATION SYSTEM is intended for use in the non-cervical spine. When used as an anterior/anterolateral and posterior, non-cervical pedicle and non-pedicle fixation system, the FRAXION SPINAL FIXATION SYSTEM is intended to provide additional support during fusion using autograft or allograft in skeletally mature patients in the treatment of the following acute and chronic instabilities or deformities: * Degenerative disc disease (DDD), defined as back pain of discogenic origin with degeneration of the disc confirmed by history and radiographic studies; * Spondylolisthesis; * Trauma, including fracture or dislocation; * Spinal stenosis; * Curvatures, including scoliosis, kyphosis, and/or lordosis; * Tumor; * Pseudoarthrosis; and * Failed previous fusion.

Device Story

Top-loading, multi-component posterior spinal fixation system; provides immobilization/stabilization of non-cervical spinal segments as adjunct to fusion. Components include polyaxial/monoaxial/cannulated/fenestrated pedicle screws, rods, setscrews, connectors, and hooks. Surgeon places screws/hooks into vertebral anatomy; seats rods into heads; tightens setscrews to lock construct. Creates rigid construct to limit motion, maintain alignment, and support load sharing during osseous fusion. Used in OR by surgeons. Benefits patient by providing mechanical stability to unstable/deformed spinal segments.

Clinical Evidence

Bench testing only. Substantial equivalence relies on mechanical evaluation data previously established for the predicate device (K212220), including testing per ASTM F1717 (static/dynamic compression bending, static torsion) and ASTM F1798 (axial/torsional grip, static flexion-extension).

Technological Characteristics

Non-powered, mechanical spinal fixation system. Materials: Ti6Al4V-ELI titanium alloy (ASTM F136). Components: polyaxial/monoaxial/cannulated/fenestrated screws, rods (5.5/6.0 mm), connectors, hooks, setscrews. Top-loading design. Supplied non-sterile, single-use. No software or energy source.

Indications for Use

Indicated for skeletally mature patients requiring non-cervical spinal fixation (anterior/anterolateral/posterior) as an adjunct to fusion (autograft/allograft) for DDD, spondylolisthesis, trauma, spinal stenosis, spinal curvatures (scoliosis/kyphosis/lordosis), tumor, pseudoarthrosis, or failed previous fusion.

Regulatory Classification

Identification

(1) Rigid pedicle screw systems are comprised of multiple components, made from a variety of materials that allow the surgeon to build an implant system to fit the patient's anatomical and physiological requirements. Such a spinal implant assembly consists of a combination of screws, longitudinal members (e.g., plates, rods including dual diameter rods, plate/rod combinations), transverse or cross connectors, and interconnection mechanisms (e.g., rod-to-rod connectors, offset connectors).(2) Semi-rigid systems are defined as systems that contain one or more of the following features (including but not limited to): Non-uniform longitudinal elements, or features that allow more motion or flexibility compared to rigid systems.

Special Controls

*Classification.* (1) Class II (special controls), when intended to provide immobilization and stabilization of spinal segments in skeletally mature patients as an adjunct to fusion in the treatment of the following acute and chronic instabilities or deformities of the thoracic, lumbar, and sacral spine: severe spondylolisthesis (grades 3 and 4) of the L5-S1 vertebra; degenerative spondylolisthesis with objective evidence of neurologic impairment; fracture; dislocation; scoliosis; kyphosis; spinal tumor; and failed previous fusion (pseudarthrosis). These pedicle screw spinal systems must comply with the following special controls:(i) Compliance with material standards; (ii) Compliance with mechanical testing standards; (iii) Compliance with biocompatibility standards; and (iv) Labeling that contains these two statements in addition to other appropriate labeling information: “Warning: The safety and effectiveness of pedicle screw spinal systems have been established only for spinal conditions with significant mechanical instability or deformity requiring fusion with instrumentation. These conditions are significant mechanical instability or deformity of the thoracic, lumbar, and sacral spine secondary to severe spondylolisthesis (grades 3 and 4) of the L5-S1 vertebra, degenerative spondylolisthesis with objective evidence of neurologic impairment, fracture, dislocation, scoliosis, kyphosis, spinal tumor, and failed previous fusion (pseudarthrosis). The safety and effectiveness of these devices for any other conditions are unknown.” “Precaution: The implantation of pedicle screw spinal systems should be performed only by experienced spinal surgeons with specific training in the use of this pedicle screw spinal system because this is a technically demanding procedure presenting a risk of serious injury to the patient.” (2) Class II (special controls), when a rigid pedicle screw system is intended to provide immobilization and stabilization of spinal segments in the thoracic, lumbar, and sacral spine as an adjunct to fusion in the treatment of degenerative disc disease and spondylolisthesis other than either severe spondylolisthesis (grades 3 and 4) at L5-S1 or degenerative spondylolisthesis with objective evidence of neurologic impairment. These pedicle screw systems must comply with the following special controls: (i) The design characteristics of the device, including engineering schematics, must ensure that the geometry and material composition are consistent with the intended use. (ii) Non-clinical performance testing must demonstrate the mechanical function and durability of the implant. (iii) Device components must be demonstrated to be biocompatible. (iv) Validation testing must demonstrate the cleanliness and sterility of, or the ability to clean and sterilize, the device components and device-specific instruments. (v) Labeling must include the following: (A) A clear description of the technological features of the device including identification of device materials and the principles of device operation; (B) Intended use and indications for use, including levels of fixation; (C) Identification of magnetic resonance (MR) compatibility status; (D) Cleaning and sterilization instructions for devices and instruments that are provided non-sterile to the end user; and (E) Detailed instructions of each surgical step, including device removal. (3) Class II (special controls), when a semi-rigid system is intended to provide immobilization and stabilization of spinal segments in the thoracic, lumbar, and sacral spine as an adjunct to fusion for any indication. In addition to complying with the special controls in paragraphs (b)(2)(i) through (v) of this section, these pedicle screw systems must comply with the following special controls: (i) Demonstration that clinical performance characteristics of the device support the intended use of the product, including assessment of fusion compared to a clinically acceptable fusion rate. (ii) Semi-rigid systems marketed prior to the effective date of this reclassification must submit an amendment to their previously cleared premarket notification (510(k)) demonstrating compliance with the special controls in paragraphs (b)(2)(i) through (v) and paragraph (b)(3)(i) of this section.

The system is used for anterior/anterolateral and posterior non-cervical pedicle and non-pedicle fixation in the treatment of acute and chronic spinal instabilities or deformities, including degenerative disc disease, spondylolisthesis, trauma, spinal stenosis, curvatures, tumor, pseudoarthrosis, and failed previous fusion. The device functions by creating a rigid spinal construct using bone-anchored screws, rods, setscrews, connectors, and hooks. Pedicle screws or hooks are placed into or onto the vertebral anatomy, longitudinal rods are seated into the screw or hook heads, and setscrews are tightened to lock the rod-screw or rod-hook interface. Once assembled, the construct limits motion between affected spinal segments, maintains alignment, supports load sharing, and provides mechanical stability during the spinal fusion process. The top- {5} K262091, Page 2 of 3 loading design allows the rod and setscrew to be inserted from above the screw head, facilitating intraoperative assembly and reducing procedural complexity. The scientific concept underlying the device is mechanical stabilization of the spine to support biological fusion. By immobilizing unstable or deformed spinal segments and maintaining the desired anatomical relationship between vertebrae, the system provides an environment that supports osseous fusion using autograft or allograft. The device does not achieve its primary intended effect by chemical action, metabolism, or pharmacological means; its function is based on mechanical fixation, load support, and stabilization. The system includes polyaxial pedicle screws, monoaxial pedicle screws, spondylolisthesis/reduction screws, cannulated screws, fenestrated screws, straight rods, pre-bent rods, transverse connectors, multiaxial transverse connectors, lateral connectors, axial extension connectors, domino connectors, laminar hooks, pedicular hooks, and setscrews. Polyaxial screws include a screw body, tulip head, and fastening component, allowing angular variability for construct alignment. Monoaxial screws have a fixed head. Cannulated screws allow placement over a guidewire. Reduction screws include an extended tulip head to assist rod reduction in spondylolisthesis cases. Rods are provided in straight and pre-bent configurations and may be contoured intraoperatively to match patient anatomy. Connectors and hooks provide additional construct options to improve fixation, accommodate anatomy, or increase construct stability. All implantable components are manufactured from titanium alloy Ti6Al4V-ELI conforming to ASTM F136. Components may be anodized for color coding and identification; anodizing forms a titanium dioxide surface layer. Components are also laser-marked for product identification. The system is supplied non-sterile, for single use only, and is available in multiple sizes to accommodate patient anatomy and surgical requirements. Significant physical characteristics include screw diameters generally ranging from 4.5 mm to 8.0 mm depending on screw type, screw lengths varying by configuration, compatibility with 5.5 mm and/or 6.0 mm rods, smooth cylindrical rods in various lengths, and threaded setscrews that lock the rod within the screw or hook head. The screw heads include U-shaped slots and threaded internal walls to receive the rod and setscrew. The screw bodies include self-tapping, tapered threaded designs to support insertion into vertebral bone. The polyaxial screw design allows angular adjustment before final locking, while the setscrew design secures the rod and minimizes the potential for misalignment or cross-threading. The principal performance characteristics of the FRAXION Spinal Fixation System are its ability to provide spinal immobilization, stabilization, construct rigidity, rod-screw locking strength, and resistance to expected mechanical loads during the fusion period. The system is designed to maintain spinal alignment, support fusion, and withstand loads associated with non-cervical spinal fixation when used according to the surgical technique and instructions for use. ## Intended Use/Indications for Use 21 CFR 807.92(a)(5) FRAXION SPINAL FIXATION SYSTEM is intended for use in the non-cervical spine. When used as an anterior/anterolateral and posterior, non-cervical pedicle and non-pedicle fixation system, the FRAXION SPINAL FIXATION SYSTEM is intended to provide additional support during fusion using autograft or allograft in skeletally mature patients in the treatment of the following acute and chronic instabilities or deformities: - Degenerative disc disease (DDD), defined as back pain of discogenic origin with degeneration of the disc confirmed by history and radiographic studies; - Spondylolisthesis; - Trauma, including fracture or dislocation; - Spinal stenosis; - Curvatures, including scoliosis, kyphosis, and/or lordosis; - Tumor; - Pseudoarthrosis; and - Failed previous fusion. ## Indications for Use Comparison 21 CFR 807.92(a)(5) The indications for use of the FRAXION Spinal Fixation System are the same as the predicate device, Artfx Spinal Fixation System (K212220). The subject device and predicate device are both intended for use in the non-cervical spine as anterior/anterolateral and posterior, non-cervical pedicle and non-pedicle fixation systems to provide additional support during fusion using autograft or allograft in skeletally mature patients for the treatment of the same acute and chronic instabilities or deformities, including degenerative disc disease, spondylolisthesis, trauma, spinal stenosis, curvatures, tumor, pseudoarthrosis, and failed previous fusion. There are no differences in indications for use between the subject device and the predicate device. In addition, the subject device is identical to the predicate device with respect to intended use, technological characteristics, design, materials, manufacturing process, performance specifications, and method of use. The predicate device manufacturer, Artfx, will manufacture the FRAXION Spinal Fixation System using the same design, materials, manufacturing processes, equipment, quality system controls, technological characteristics, and performance specifications as the device cleared under K212220. {6} K262091, Page 3 of 3 Therefore, the indications for use are the same as the predicate device and do not constitute a new intended use. ## Technological Comparison 21 CFR 807.92(a)(6) The FRAXION Spinal Fixation System has the same technological characteristics as the predicate device, Artfx Spinal Fixation System (K212220). The subject device and predicate device have the same design, materials, chemical composition, principle of operation, sterility status, method of use, and performance specifications. Both devices are top-loading, multiple-component spinal fixation systems consisting of pedicle screws, rods, connectors, hooks, and setscrews. The subject device uses the same rod-screw-setscrew interconnection mechanism as the predicate device. After the screws are placed in the spine, the rod is seated into the U-shaped slot of the screw head and locked using a setscrew to create a spinal fixation construct. The subject device and predicate device are manufactured from the same implant material, Ti6Al4V-ELI titanium alloy conforming to ASTM F136. The material composition, biocompatibility profile, corrosion resistance, mechanical strength, and fatigue strength are the same as the predicate device. The device is non-powered and does not use an energy source. Its principle of operation is mechanical stabilization and immobilization of non-cervical spinal segments as an adjunct to fusion. The function of the subject device is the same as the predicate device: to act as a spinal implant construct to stabilize and promote spinal fusion. The subject device is identical to the predicate device cleared under K212220 with respect to design, materials, technological characteristics, interconnection mechanism, performance specifications, and intended use. Therefore, the FRAXION Spinal Fixation System has the same technological characteristics as the predicate device. No technological differences exist that raise different questions of safety or effectiveness. ## Non-Clinical and/or Clinical Tests Summary & Conclusions 21 CFR 807.92(b) The FRAXION Spinal Fixation System is identical to the predicate device, Artfx Spinal Fixation System (K212220), with respect to intended use, indications for use, design, materials, technological characteristics, principle of operation, interconnection mechanism, manufacturing approach, sterility status, and performance specifications. Therefore, the nonclinical performance information relied upon for substantial equivalence is based on the previously established performance data for the predicate device and the mechanical evaluation provided in K212220, including ASTM F1717 (static and dynamic compression bending, static torsion) and ASTM F1798 (axial grip, torsional grip, static flexion-extension). The information presented demonstrates the substantial equivalence of the subject device.
