## Device Story

Leksell® Vantage Stereotactic System is a mechanical neurosurgical instrument for coordinate referencing and skull fixation. Device comprises a head frame and semi-circular arc (190 mm radius). It uses Cartesian (x, y, z) coordinates to triangulate brain targets. Fiducial markers are registered to intracranial images (CT, MR, X-ray) or brain atlases. Used in access-controlled healthcare facilities by neurosurgeons. Provides support/guidance for surgical instruments (biopsy needles, electrodes, etc.) to reach targets. X-ray accessories (fiducial box, table fixations) enable target localization during angiography. System ensures mechanical accuracy of 0.9 mm. We have reviewed your section 510(k) premarket notification of intent to market the device referenced above and have determined the device is substantially equivalent (for the indications for use stated in the enclosure) to legally marketed predicate devices marketed in interstate commerce prior to May 28, 1976, the enactment date of the Medical Device Amendments, or to devices that have been reclassified in accordance with the provisions of the Federal Food, Drug, and Cosmetic Act (the Act) that do not require approval of a premarket approval application (PMA). You may, therefore, market the device, subject to the general controls provisions of the Act. Although this letter refers to your product as a device, please be aware that some cleared products may instead be combination products. The 510(k) Premarket Notification Database available at https://www.accessdata.fda.gov/scripts/cdrh/cfdocs/cfpmn/pmn.cfm identifies combination product submissions. ### VIII. Summary of Performance Testing (Non-Clinical)

Verification activities have been performed to demonstrate that the X-ray accessories work as intended according to the system requirements for the Leksell Vantage Stereotactic System and its relevant subsystem requirements and that there is no unforeseen side-effect to the existing product base of Leksell Vantage Stereotactic System resulting in the changes made.

Performance testing, including design and usability validation of the system, has been performed by competent and professionally qualified personnel to ensure that the product fulfils the intended use and user needs.

Results from verification and validation testing demonstrate that conformance to applicable technical requirement specification, user needs have been met and that the device functions as intended.

The new X-ray accessories have also been evaluated for biocompatibility.

### IX. Summary of Performance Testing (Clinical)

No animal or clinical tests were performed to establish substantial equivalence with the predicate devices. The performance data demonstrate that the subject device is as safe and effective and performs as well as the predicate devices (K190887 and K152558).

### X. Substantial Equivalence Conclusion

The intended use, the basic functionality, and the fundamental technological features of the Leksell Vantage Stereotactic System did not change.

The subject device remains as safe and effective as the primary predicate device (K190877) and predicate device (K152558).

The subject device does not raise different questions of safety and effectiveness than the predicate devices and safety and effectiveness is demonstrated with the same methods (FDA guidance and recognized consensus standards) and with the same results of the predicate devices. This shows that the subject device is substantially equivalent to the primary predicate device (K190877) in intended use, principles of operation, technological characteristics, performance, and labelling. Also, the subject device is also substantially equivalent to predicate device (K152558) regarding usage of X-ray accessories.
