## Indications for Use

The Pisces-SC Standalone Cervical Interbody System is intended to be used with or without two screws and/or anchors which accompany the interbodies. When used with two screws, the devices are intended for standalone use (requiring no supplemental fixation) at one or two contiguous levels. When used with one or more anchors or without integrated fixation (screws / anchors), these devices require additional supplemental fixation (i.e. anterior cervical plate or posterior cervical screws).

## Device Description:

The Pisces-SC Standalone Cervical Interbody System is a standalone interbody fusion device for the cervical spine that may be used with two titanium alloy screws or anchors which accompany the interbodies. When used with screws, these devices are standalone interbody fusion devices. When used with anchors or without screws, these devices are intended for use with supplemental fixation (e.g. anterior cervical plate or posterior cervical screws). The Pisces-SC interbodies are available in multiple sizes and are manufactured from titanium alloy (Ti-6Al-4V ELI) per ASTM F3001. The interbodies are single use devices, which are sterilized via gamma radiation and are provided to the user in sterile packages. The screws and anchors are single-use devices which are available in multiple sizes and are manufactured from titanium alloy (Ti-6Al-4V ELI) per ASTM F136. The locking plates are single-use devices which are available in a single size and are manufactured from titanium alloy (Ti-6Al-4V ELI) per ASTM F136. The instruments used to insert the implants are manufactured from medical grade stainless steel. The screws, anchors, locking plates, and instruments are provided non-sterile and must be sterilized prior to use in a supplied re-usable tray for steam sterilization. The sterilization parameters have been validated to achieve a SAL of 10⁻⁶.

The Pisces-SC Standalone Cervical Interbody System contains components manufactured using traditional machining methods and additive manufacturing methods.

The Pisces-SC interbodies are available in three different footprints (14x12mm, 16x13mm, 18x14mm), heights ranging from 5mm to 12mm in 1mm increments, and lordotic angles ranging from 0° to 12°. The Pisces-SC screws and anchors have a neutral trajectory angle of 35° cephalad-caudal. The Pisces-SC locking plate assembly is comprised of a plate that prevents backout of the fixation components (screws or anchors) with an integrated central M2x0.4 bolt that affixes the locking plate assembly to the interbody. The Pisces-SC locking plate assembly is available in one size that is compatible

with all interbody configurations. The locking plate is 4.25mm tall and tapers down to 3.4mm over a 12mm width. The back of the plate has features that key into the interbody to create a secure interface. The plate also has two through-holes that allow for engagement of the locking plate inserter.

The uninterrupted endplate graft contact area varies based on the footprint of the device. The interbody with the smallest footprint and shortest height has approximately 123mm² of uninterrupted endplate graft contact area and approximately 307mm³ of uninterrupted graft volume. On the spacer there is a centrally located threaded hole to connect with the inserter and locking plate.

The results support that the subject device is substantially equivalent to the predicates.

### Conclusion:

Based on Indications for use, technological characteristics, and comparison with the predicate devices, the subject device has demonstrated substantial equivalence.
