## Device Story

Permatage Flowable, Settable Hemostatic Bone Paste is a sterile, nonabsorbable, two-component paste used to control bleeding from cut or damaged bone. The device consists of granular calcium phosphate, paraffin oil, vitamin E acetate, a triglyceride, and polyether-based polymers. Components are supplied in a dual-barrel cartridge and mixed immediately prior to use. Once applied to the bone surface, the material hardens into a cohesive, nonabsorbable barrier that adheres to the bone. The device is intended for manual application by a clinician during surgery. It functions as a mechanical tamponade to stop bleeding without significant exothermic reaction. The hardening process occurs within 10 minutes, with a 2-minute working time. The device benefits patients by providing an effective, biocompatible hemostatic seal on bone surfaces.

## Clinical Evidence

No clinical studies were conducted. Evidence is based on bench testing and animal studies. Bench testing included visual inspection, paste stiffness, package leak testing, temperature sensitivity, water uptake/swelling/dissolution, and endotoxicity (USP <85>). Hemostasis performance was evaluated in animal models and found equivalent to the predicate. Sterilization was validated per ISO 11137-1:2015 and ISO 11137-2:2015.

We have reviewed your section 510(k) premarket notification of intent to market the device referenced above and have determined the device is substantially equivalent (for the indications for use stated in the enclosure) to legally marketed predicate devices marketed in interstate commerce prior to May 28, 1976, the enactment date of the Medical Device Amendments, or to devices that have been reclassified in accordance with the provisions of the Federal Food, Drug, and Cosmetic Act (the Act) that do not require approval of a premarket approval application (PMA). You may, therefore, market the device, subject to the general controls provisions of the Act. Although this letter refers to your product as a device, please be aware that some cleared products may instead be combination products. The 510(k) Premarket Notification Database available at https://www.accessdata.fda.gov/scripts/cdrh/cfdocs/cfpmn/pmn.cfm identifies combination product submissions. ## Biocompatibility Risk Assessment

Each of the tests conducted passed the requirements as stated in the protocols and in recognized standards. The materials from which the Orthocon device is fabricated have an established history of use, and the devices have been tested in accordance with applicable FDA guidelines. The data presented demonstrate that the device is suitable for its indicated use. Any differences between Permatage and the predicate do not raise new concerns or risks.
