Indications for Use

The APERTO Lucent System is an imaging device and is intended to provide the physician with physiological and clinical information, obtained non-invasively and without the use of ionizing radiation. The MR system produces transverse, coronal, sagittal, oblique, and curved cross sectional images that display the internal structure of the head, body, or extremities. The images produced by the MR system reflect the spatial distribution of protons (hydrogen nuclei) exhibiting magnetic resonance. The NMR properties that determine the image appearance are proton density, spin-lattice relaxation time (T1), spin-spin relaxation time (T2) and flow. When interpreted by a trained physician, these images provide information that can be useful in diagnosis determination.

Device Story

The APERTO Lucent MRI System is a 0.4T permanent magnet MRI scanner used in clinical settings by trained physicians and technologists. It acquires MR signals (protons) via RF excitation and magnetic field gradients to generate cross-sectional anatomical images. The system includes new software (V11.0C) featuring 'DLR Plus' (a deep learning-based image reconstruction algorithm) for noise reduction and image enhancement, and 'AutoPose' for automated slice positioning. Input data consists of raw MR signal echoes; the system processes these into 2D/3D images. Physicians interpret these images to assist in clinical diagnosis. The device benefits patients by providing high-quality diagnostic imaging without ionizing radiation. Hardware upgrades include increased memory (32GB) and SSD storage.

Clinical Evidence

Clinical validation included 68 subjects (ages 7-89) for DLR Plus and 274 cases for AutoPose. DLR Plus performance was evaluated by three board-certified radiologists using a 3-point scale; results showed statistically significant improvement (p<0.05) in SNR, truncation artifacts, sharpness, and lesion conspicuity compared to conventional images. All images were deemed clinically acceptable. AutoPose performance was validated by radiological technologists, demonstrating successful automated slice positioning for spine, knee, shoulder, hip, and pelvis exams.

Technological Characteristics

0.4T permanent magnet; 16.18MHz resonant frequency; 22mT/m gradient strength; 55 T/m/sec slew rate. Hardware includes 32GB RAM and SSD storage. Complies with NEMA MS 1-5, 8, 14 and IEC 60601-1, 60601-1-2, 60601-2-33, 62304. Software V11.0C includes deep learning-based reconstruction (DLR Plus) and automated positioning (AutoPose).

Indications for Use

Indicated for patients requiring non-invasive diagnostic imaging of the head, body, spine, and extremities using proton-based MR imaging (T1, T2, proton density, diffusion-weighted, and MRA).

The most ubiquitous of these nuclei is hydrogen, which makes it the primary nuclei currently used in magnetic resonance imaging. When placed in a static magnetic field, these nuclei assume a net orientation or alignment with the magnetic field, referred to as a net magnetization vector. The introduction of a short burst of radiofrequency (RF) excitation of a wavelength specific to the magnetic field strength and to the atomic nuclei under consideration can cause a re-orientation of the net magnetization vector. When the RF excitation is removed, the protons relax and return to their original vector. The rate of relaxation is exponential and varies with the character of the proton and its adjacent molecular environment. This re-orientation process is characterized by two exponential relaxation times, called T1 and T2. A RF emission or echo that can be measured accompanies these relaxation events. The emissions are used to develop a representation of the relaxation events in a three dimensional matrix. Spatial localization is encoded into the echoes by varying the RF excitation, applying appropriate magnetic field gradients in the x, y, and z directions, and changing the direction and strength of these gradients. Images depicting the spatial distribution of the NMR characteristics can be reconstructed by using image processing techniques similar to those used in computed tomography. ### Physical and Performance Characteristics MRI is capable of producing high quality anatomical images without the associated risks of ionizing radiation. The biological properties that contribute to MR image contrast are different from those responsible for x-ray image contrast. In MR imaging, difference in proton density, blood flow, and T1 and T2 relaxation times can all contribute to image contrast. By varying the pulse sequence characteristics, the resulting images can emphasize T1, T2, proton density, or the molecular diffusion of water or other proton containing molecules. ### Performance Evaluation The APERTO Lucent MRI System is equivalent to the APERTO Lucent MRI System (K253862) with following exceptions: - The storage type of the console is changed from HDD to SSD, and the memory capacity is changed from 16GB to 32GB. - Application software is changed to V11.0C. The validation results of the new features using machine learning (DLR Plus and AutoPose) were described below. Performance tests of DLR Plus were conducted by using digital and physical phantoms in terms of SNR, truncation artifacts, image sharpness, and image contrast. The phantom testing demonstrated that DLR Plus could reduce noise and truncation artifacts and could improve image sharpness in the image by using the metrics of SNR, Total Variation, and Relative Edge Sharpness. The phantom testing also demonstrated that DLR Plus did not degrade the image quality by using the metric of Contrast Change Rate. In addition, three U.S. board-certified radiologists reviewed the clinical and volunteer images with DLR Plus in terms of the image quality metrics (SNR, truncation artifacts, image sharpness, lesion conspicuity, and overall image quality). The images reconstructed either without or with DLR Plus were randomized, blinded to the reviewers, and compared by the reviewers in terms of the image quality metrics using a 3-point scale. All images used for this comparison were also evaluated by the reviewers in terms of clinical acceptability. The 68 unique subjects (patients and volunteers) from U.S. and ©2026 FUJIFILM Healthcare Americas APERTO Lucent MRI System Page 9 of 10 {13} 510(k) Summary Japan (Male:37, Female:31, Age:7~89 years, BMI:14.2~42.5) were scanned in the anatomical regions from brain to ankle in order to provide the test datasets separately from the training and validation datasets using the FUJIFILM 0.4T MRI Scanner (APERTO Lucent). The total of 171 cases in multiple orientations (axial, sagittal, and coronal), multiple dimensions (2D and 3D), and various contrast weightings (T1-/T2-/T2*-/PD-weighted image, FLAIR, DWI, and MRA) were obtained for the test dataset by pulse sequences of SE, FSE, GE, SSFP, DW EPI, TOF, and PC. The test dataset was independent of the training and validation datasets. The review results indicated that all image quality metrics (SNR, truncation artifacts, image sharpness, lesion conspicuity, and overall image quality) in the images with DLR Plus were superior to those in the conventional images with statistically significant differences (p<0.05). Additionally, all images with DLR Plus were evaluated as clinically acceptable by the reviewers. In conclusion, the phantom testing and the clinical image testing demonstrated that DLR Plus could reduce noise and truncation artifacts, could improve image sharpness, and did not degrade the image quality compared to the predicate device. The performance tests of AutoPose Spine, Shoulder, Knee, HipJoint, Pelvis for male, and Pelvis for female were conducted by the three certified radiological technologists. The evaluation results showed that the almost cases in AutoPose Spine, Shoulder, Knee, HipJoint, Pelvis for male, and Pelvis for female were able to set the slice positions for a scan without manual adjustment. The evaluation results also showed that the remaining cases were the same user operation steps as the manual slice positioning as without using AutoPose. The information on the data in the evaluation is shown below. | | Spine | Knee | Shoulder | HipJoint | Pelvis for female | Pelvis for male | | --- | --- | --- | --- | --- | --- | --- | | Number of cases | 112 | 35 | 32 | 32 | 33 | 30 | | Data acquisition site | FUJIFILM Corp., FUJIFILM Healthcare Americas Corp., two clinical sites in U.S. | | | | | FUJIFILM Corp., FUJIFILM Healthcare Americas Corp. | | Subject Type | Healthy volunteer and patients | | | | | Healthy volunteer | As a result of the analysis: | Testing Type | Rationale Analysis | | --- | --- | | Performance Testing - Clinical | Clinical image examples are provided for applicable new feature and that we judged to be sufficient to evaluate clinical usability. In addition, a radiologist validated that the clinical images have acceptable image quality for clinical use. | ## Conclusions The APERTO Lucent MRI system is substantially equivalent with respect to hardware, software, safety, effectiveness, and functionality to the APERTO Lucent MRI System (K253862). ©2026 FUJIFILM Healthcare Americas APERTO Lucent MRI System Page 10 of 10
