Device Story

Endoform Dermal Template is a sterile, single-use, 100% intact extracellular matrix (ECM) derived from ovine forestomach tissue; contains collagen I and III. Device is a lyophilized sheet; rehydrates with wound exudate or saline to form a soft, conforming sheet for wound application. Used in clinical settings for wound management; incorporated into wound over time. Subject device expands size range up to 1000 cm² (previously 400 cm²) and utilizes double-pouch Tyvek/film packaging. No change to material, manufacturing, or clinical function.

Conclusion** The subject device Endoform Dermal Template is substantially equivalent to the predicate device (K092096) with respect to indications for use, overall design and technological characteristics.
