We have reviewed your section 510(k) premarket notification of intent to market the device referenced above and have determined the device is substantially equivalent (for the indications for use stated in the enclosure) to legally marketed predicate devices marketed in interstate commerce prior to May 28, 1976, the enactment date of the Medical Device Amendments, or to devices that have been reclassified in accordance with the provisions of the Federal Food, Drug, and Cosmetic Act (the Act) that do not require approval of a premarket approval application (PMA). You may, therefore, market the device, subject to the general controls provisions of the Act. Although this letter refers to your product as a device, please be aware that some cleared products may instead be combination products. Device Description

The SEISMIQ Intravascular Lithotripsy System is a proprietary balloon catheter and console designed to enhance percutaneous transluminal angioplasty by delivering calcium disrupting lithotripsy prior to balloon dilatation at low pressures. The application of lithotripsy mechanical pulse waves alters the structure of an occlusive vascular deposit (stenosis) allowing low-pressure balloon dilatation of the stenosis and facilitates the passage of blood.

The SEISMIQ IVL Catheter is delivered through the peripheral arterial system of the lower extremities to the lesion site. The balloon is partially inflated, and the lithotripsy emitters generate pulsatile mechanical energy within the balloon at the target treatment site allowing subsequent dilatation of a peripheral artery stenosis using low balloon pressure. The SEISMIQ IVL Catheter is a single-use device supplied sterile to the customer.

SEISMIQ™ IVL System

The SEISMIQ IVL Console delivers energy through the integrated catheter cabling to the emitters located inside the catheter balloon in the SEISMIQ IVL Catheter. The SEISMIQ IVL Console is a non-sterile, reusable device.

## Technological Comparison

This 510(k) Premarket Notification is being submitted for minor modifications to the SEISMIQ™ IVL System. The product line extension adds additional catheter sizes with a smaller ballon diameter and length to the SEISMIQ IVL System portfolio.

To increase the size range of the system, the additional catheter sizes include a smaller ballon diameter and length as compared to catheters cleared under K250225. Differences between the two catheters do not raise new questions of safety and effectiveness.

The SEISMIQ IVL System has the same intended use principles of operation and has substantially equivalent technological characteristics, including the same fundamental scientific technology, design, energy source, shelf life and sterilization as the predicate IVL System.

## Summary of Performance Data

Objective evidence demonstrating that the SEISMIQ IVL System design output meets the product design input requirements as well as that device performance characteristics conform to user needs and intended uses as defined in the product specification was provided. Testing was conducted in accordance with Bolt Medical's risk analysis procedures, and all applicable FDA guidance documents and relevant international standards. Testing included:

Results demonstrate that the performance of the SEISMIQ IVL System meets its design specifications and demonstrates substantial equivalence for its intended use.

## Summary of Biocompatibility Testing

Biocompatibility testing was conducted on the predicate device (K250225) in accordance with ISO 10993-1:2020 and FDA Guidance Use of International Standard ISO 10993-1, "Biological evaluation of medical devices - Part 1: Evaluation and testing within a risk management process" and leveraged for the subject device.

## Summary of Animal Testing

A chronic GLP study was conducted in a porcine in vivo model to determine the safety of delivering pulsatile mechanical energy to peripheral arteries with the Bolt IVL System, and submitted as part of K250225. There have been no modifications made to the subject device that impact the delivered therapy of the SEISMIQ IVL System. Therefore, data from K250225 is leveraged for the subject device submission.

## Summary of Clinical Data

Clinical evidence which evaluated the safety and performance of the Bolt IVL System was submitted as part of K250225 for above the knee (RESTORE ATK) and below the knee (RESTORE BTK), and is leveraged for the subject device submission. There have been no modifications made to the subject device that impact the delivered therapy of the SEISMIQ IVL System.

## Basis for Substantial Equivalence

The SEISMIQ IVL System shares the same intended use and mechanism of action as well as similar technological characteristics as the predicate device. Any differences between the SEISMIQ IVL System and the predicate device were evaluated through bench testing which demonstrated acceptable device performance and confirmed that there are no new questions of safety or effectiveness. Therefore, the SEISMIQ IVL System is substantially equivalent to the predicate device.
