Indications for Use

Immunoassay for the in vitro qualitative detection of IgG antibodies to Epstein-Barr virus Nuclear Antigen-1 (EBNA-1) in human serum and plasma. When performed in conjunction with other Epstein-Barr virus markers, this assay is intended for use as an aid in the diagnosis of Epstein-Barr virus infection, such as infectious mononucleosis. The electrochemiluminescence immunoassay “ECLIA” is intended for use on cobas e immunoassay analyzers.

Device Story

Elecsys EBV EBNA IgG is a fully automated, qualitative electrochemiluminescence immunoassay (ECLIA) for use on cobas e 801 analyzers. It detects IgG antibodies to EBV Nuclear Antigen-1 (EBNA-1) in human serum or plasma. The assay uses a sandwich principle: biotinylated EBNA-1 antigen and ruthenium-labeled EBNA-1 antigen form a complex with sample antibodies, which is then captured by streptavidin-coated microparticles. Magnetic capture and voltage application induce chemiluminescent emission, measured by a photomultiplier. Results are calculated automatically by comparing the sample signal to a cutoff value (COI < 1.0 non-reactive; COI ≥ 1.0 reactive). The device is intended for clinical laboratory use to aid in diagnosing EBV infection. It provides rapid, high-throughput testing (18-minute test time) to assist clinicians in determining patient EBV serological status.

Clinical Evidence

Clinical performance was evaluated in a multisite study using 974 specimens (720 prospective, 253 retrospective). The study compared the Elecsys EBV EBNA IgG assay against a composite consensus comparator. In prospective adult cohorts, PPA was 100% (95% CI: 99.2-100%) and NPA was 60.9% (95% CI: 45.4-74.9%). In prospective pediatric cohorts, PPA was 97.8% (95% CI: 92.1-99.7%) and NPA was 94.1% (95% CI: 88.7-97.4%). Retrospective cohorts showed varying agreement based on serological status. Overall, the data supports the device's use as an aid in diagnosis.

Performance not established for immunocompromised/immunosuppressed patients, cord blood, neonates, or infants.

Regulatory Classification

Identification

Epstein-Barr virus serological reagents are devices that consist of antigens and antisera used in serological tests to identify antibodies to Epstein-Barr virus in serum. The identification aids in the diagnosis of Epstein-Barr virus infections and provides epidemiological information on diseases caused by these viruses. Epstein-Barr viruses are thought to cause infectious mononucleosis and have been associated with Burkitt's lymphoma (a tumor of the jaw in African children and young adults) and postnasal carcinoma (cancer).
