The Sofia 2 SARS Antigen+ FIA is a lateral flow immunofluorescent sandwich assay that is used with the Sofia 2 instrument for the rapid, qualitative detection of SARS-CoV-2 nucleocapsid protein antigens directly in anterior nasal swab specimens from individuals with signs and symptoms of upper respiratory infection. The test is intended for use as an aid in the diagnosis of SARS-CoV-2 infections (COVID-19) in symptomatic individuals when either: tested at least twice over three days with at least 48 hours between tests; or when tested once, and negative by the Sofia 2 SARS Antigen+ FIA and followed up with a molecular test. The test does not differentiate between SARS-CoV and SARS-CoV-2. A negative test result is presumptive, and does not preclude SARS-CoV-2 infection; it is recommended these results be confirmed by a molecular SARS-CoV-2 assay. Positive results do not rule out co-infection with other respiratory pathogens and should not be used as the sole basis for diagnosis, treatment, or other patient management decisions. Performance characteristics were established from August 2021 to November 2022 when SARS-CoV-2 Delta and Omicron were the predominant SARS-CoV-2 variants in circulation. When other variants are emerging, performance characteristics may vary. The Sofia 2 Lyme FIA employs immunofluorescence for the rapid differential detection of human IgM and IgG antibodies to Borrelia burgdorferi from finger-stick whole blood specimens from patients suspected of B. burgdorferi infection of at least 2 weeks' duration. The Sofia 2 Lyme FIA is intended for use as an aid in diagnosis of Lyme disease. A negative result does not preclude infection with B. burgdorferi. Positive results must be confirmed by testing with a corresponding second-tier B. burgdorferi Western blot assay. Test results are to be used in conjunction with information obtained from the patient's clinical evaluation and other diagnostic procedures. The assay is to be performed on the Sofia 2 instrument. Professional guidelines should be consulted regarding testing and treatment for Lyme disease when acute B. burgdorferi infection is suspected.

#### 4. Detection Limit and Assay Reportable Range:
K254264 - Page 8 of 15
{8}
# a) Sofia 2 SARS Antigen+ FIA:
The purpose of this study was to demonstrate that the Limit of Detection (LoD) is equivalent for the two software versions of the Sofia 2 analyzer with the Sofia 2 SARS Antigen+ FIA in nasal samples with heat inactivated SARS-CoV-2 virus. The limit of detection was established in two (2) phases using dilutions in negative nasal matrix (NNM) At least five (5) analyzers with each version of the software were used in which all test replicates in both phases were alternatively read in each Sofia 2 analyzer in the READ NOW mode.
LoD was determined in a 2-phase process. In the first phase (range-finding), a 1:5 dilution series was prepared by mixing heat inactivated SARS-CoV-2 stock solution directly into NNM and each dilution was tested using one (1) Sofia 2 SARS Antigen+ FIA lot on both software versions of the Sofia 2 analyzer. The preliminary LoD was determined by the lowest viral concentration that was positive in 5 out of 5 replicates (i.e., 100% detection) on the Sofia 2 analyzer.
In phase 2 (confirmation), the preliminary LoD concentration and 2-fold dilutions above and below this value were tested in replicates of twenty (20) using both versions of the Sofia software on one (1) lot of cassettes to confirm the LoD concentration. If the preliminary LoD was not confirmed, additional levels were tested until the lowest viral concentration that was positive in at least 19 of the 20 replicates (≥ 95% positivity) was determined.
The overall results are shown in Table 1. Identical LoD results were observed with both versions of the software.

# b) Sofia 2 Lyme FIA:
To assess the equivalency of the two software versions of the Sofia 2 analyzer, the performance of the Sofia 2 Lyme FIA was evaluated with different dilutions of recombinant IgG and IgM antibodies to B. burgdorferi in negative whole blood matrix. The negative matrix consisted of individual whole blood samples collected from donors confirmed negative using the Sofia 2 Lyme FIA. Initially, five (5) replicates were tested for each of the serial dilutions on one lot of Sofia Lyme FIA.
K254264 - Page 9 of 15
{9}
Additional confirmatory testing was performed using 20 replicates with concentrations near the assay cut-off. Testing was performed on ten Sofia 2 analyzers (five per software version).
Results demonstrated equivalent performance of Sofia 2 Lyme FIA for IgG and IgM analytes using the Sofia 2 analyzer with both software versions. The data is summarized in Tables 2-3.

For all samples tested in this study, there was 100% concurrence observed between both versions of the Sofia software.
b) Sofia 2 Lyme FIA
The purpose of this study was to demonstrate comparable performance for both versions of the Sofia 2 software with the Sofia 2 Lyme FIA in whole blood samples with Lyme IgG and IgM.
Negative matrix collected from 53 individual negative donors (confirmed negative on Sofia 2 v1) was incorporated into a panel consisting of 50 positive samples that were spiked with either Lyme IgG or IgM (single analyte samples) at ranges from 1 to 10x LoD of the panel analyte and 30 negative samples (whole blood with no analyte).
Samples were tested side by side to demonstrate comparable performance between the Sofia 2 software version 1 and Sofia 2 software version 2 analyzers.

All IgG positive samples yielded 100% agreement to the expected result with the Sofia 2 analyzer and software version 2. One (1) invalid result (3-5x LoD IgM) was observed during the study on both software version 1 and version 2. Further analysis of the cassette image showed no sample migration along the test strip, likely due to aggregation. The retest was valid and yielded the expected result. Twenty-four (24) unexpected IgG positive results were observed among the IgM positive samples. Of these, twenty-three (23) were observed on both software version 1 and version 2, while one (1) was observed only on software version 2. The presence of unexpected IgG positive results does not impact patient treatment, as an initial positive result triggers a confirmatory serology test at a reference laboratory.

Performance Near the Assay Cutoff
i. Sofia 2 SARS Antigen+ FIA
The purpose of this study was to demonstrate that the performance near the cut-off is equivalent for both versions of the Sofia 2 software with the Sofia 2 SARS Antigen+ FIA in nasal samples with heat inactivated SARS-CoV-2 virus.
A coded panel of individual contrived samples was tested to demonstrate that multiple operators can consistently and correctly perform the assay as intended on both software versions of the Sofia 2 analyzers using a single (1) assay lot. Testing of this panel considered multiple analyzers, operators, days, and sample types to obtain variability

assessment as well as collect comparison data between the two systems. The contrived panel consisted of:
- Negative Sample (no analyte, C₀)
- Low Positive (1-2x LoD)
- Moderate Positive (4x LoD)
Each sample level was tested in triplicate (n=3) across ten (10) different testing events (2 events per day over 5 days) at the manufacturer's internal site with at least two (2) operators and ten (10) analyzers for use during the study. Each sample was tested for a total of thirty (30) times over the course of the study. One (1) kit lot was run and tested per event. A calibration check of the Sofia 2 analyzers was performed prior to the beginning of the study. One (1) set of Positive and Negative Control Swabs were tested for each day of the study on each analyzer.
The overall study results are shown in Table 4 below. Equivalent performance (negative and positive percent agreement) was observed between both versions of the Sofia 2 analyzer software.

Part 2 — Embeddings: the galaxy map 3 min
1. Search: exact and fuzzy
Type a phrase like "coronary artery calcification" into the search box. You get two kinds of results. Exact results match the literal phrase — prefix searches work ("coronary artery calcificati") but suffix searches do not. Fuzzy results match on the meaning and intent of your phrase rather than the exact words, and are sorted by relevance score. Hover over the Exact or Fuzzy badge on any row to see exactly why it matched.
Use the checkboxes above the results to narrow: SaMD keeps only Software as a Medical Device, AI / ML keeps only devices with AI.
Exact vs. fuzzy search: what's the difference?
Exact matches on the literal phrase (prefix search works, suffix does not). Fuzzy matches on the meaning and intent of the phrase rather than the exact words. Hover over the badge on any row to see why it matched.
You search "coronary artery calcification" and want only software devices with AI. What two filters do you apply?
Narrow by SaMD (Software as a Medical Device), then narrow by AI/ML (devices with AI).
2. The results table
Scroll right in the results table. The intended use is extracted for you — no need to open the PDF. The device story gives a high-level snapshot of what the device does and how it's used. The AI Performance sub-table shows each output name, acceptance criteria, observed values, and development/test dataset descriptions — the same format Innolitics uses for regulatory strategy outputs, and the fastest high-level fingerprint of an AI device. It is AI-generated but has been very reliable in practice.
Where do you find a device's intended use without opening the PDF?
Scroll right in the search results table. The intended use column is extracted for you; no need to dig into the 510(k) summary PDF.
What does the AI Performance sub-table show, and why is it useful?
Output name, acceptance criteria, observed values, development dataset description, and test dataset description. It's the same format we use for regulatory strategy output and Fast 510(k) input, and the fastest high-level fingerprint of an AI device. AI-generated but reliable in practice.
3. Judging fuzzy relevance
Fuzzy results trail off in relevance as you scroll. Use three signals to decide how far down to go: the fuzzy badge explanations, the intended use column, and whether your target output (e.g., Cobb angle) still appears in the AI Performance sub-table. Once it stops appearing, you're past the relevant zone. A top hit with a low score (~0.4) and a stretched explanation is a hint the closest predicates are far away — the project may be headed for De Novo. Note the fuzzy search is a pattern match: it doesn't handle negation ("not") well, and hardware devices can appear — filter by SaMD/AI ML to cut them.
How do you judge how far down fuzzy search results to go?
Use the relevancy signals: the fuzzy badge explanations, the intended use column, and whether the target output (e.g., Cobb angle) still appears in the AI Performance sub-table. Once it stops appearing, results are trailing off in relevancy.
4. Device detail page: chat and citations
Click a device name to open its detail page: device facts on the left, a chat window on the right. Ask something like "Describe the training data". The answer carries little citation bubbles — click one to jump to the highlighted passage in the source PDF, so you can verify every AI answer against the document. There's also a Download PDF button for sharing.
How do you verify an AI chat answer on the device detail page?
Click the citation bubbles to jump to the relevant highlight in the source document.
Reading rule for every project: how many summaries do you read in full?
At least the three most relevant 510(k) or De Novo summaries, in full. After that, use targeted chat questions to confirm your memory quickly. The tool supports this professional habit — it doesn't replace it.
5. Side-by-side comparison
Select multiple rows in the results table (aim for under ~10), then open the PDF Viewer tab. Ask one question — it goes to all selected devices in parallel, each with citations. This is the fastest way to compare and contrast devices: training data, PCCP scope, how they handled adding new scanners, and so on.
What does the side-by-side PDF viewer mode do?
Select multiple devices, open the PDF viewer tab, and ask one question (e.g., "Describe the training data"). It queries all selected devices simultaneously with citations, so you can compare and contrast quickly.
6. Collections
With rows selected, go to the Collections tab and create a labeled collection (e.g., "Cobb Angle Project"). Reload that selection any time — before a client call, pull up the collection and ask questions across all of its devices at once.
How do you save a set of selected devices for later use?
Select the rows, go to the Collections tab, and create a labeled collection (e.g., "Cobb Angle Project"). You can reload the selection anytime and carry it into the PDF viewer and other tabs that support selections.
7. Product codes and the regulations tree
Click a product code in the results to jump to it in the regulations tree — identification text, sibling product codes, and devices you can open in a PDF viewer on the right. Click a regulation number to see its identification, special controls, and related product codes. You can also search by product code or regulation number at the top of the tree. Always read the special controls if any exist for your device — it broadens your search and sharpens pre-kickoff research.
What can you do from the regulations tree view?
Browse product codes and regulation numbers, read the identification text and special controls, browse sibling product codes, open device PDFs on the right, and search by product code or regulation number at the top of the tree.
8. Chart view
Click Show Chart and segment by regulation number (or product code) to see which regulations dominate your result set. Clicking a regulation takes you into the regulations tree. Great for spotting that most matches are, say, hardware laparoscopic devices — a cue to go back and filter.
How do you see which regulations dominate a search result set?
Click "Show Chart" and segment by Regulation Number. Clicking a regulation takes you to the regulations tree.
9. The predicate graph
Open the Predicates tab for a family-tree view of predicate relationships. Click a node to trace its parents and children; selections from search carry over pre-selected. Commonly predicated devices are worth reading — a lot of people predicated them for a reason. The visual lineage is also handy on client calls, e.g. to show how a predicate family evolved and justify why your predicate still holds.
In the predicate graph, why are commonly predicated devices worth reading?
A lot of people predicated them for a reason. Clicking a node traces parents and children, and selections from search carry over pre-selected.
10. Embeddings: the galaxy map
The Embeddings tab plots every matching document in a 2-D "galaxy map" where semantically similar devices cluster together. Hover or click clusters to explore, and let AI label the clusters for you. Embeddings beat product codes for grouping: two devices can carry different product codes (LLZ vs. QIH) yet do the same thing — the embedding captures the meaning of the intended use and device story. This is also exactly how retrieval-augmented generation (RAG) works under the hood, and it makes a great visual on client calls.
Try it yourself
Head to the search page and work through a few of these AI/ML fuzzy searches to build intuition: perivascular fat on CT · aortic valve calcification opportunistic screening on noncontrast CT · breast cancer prediction on digital pathology slides · autism detection · gestational age prediction · a hearing aid that can also detect a pulse · foundation model based analysis of ECG · large language models · penetration test. Watch how the relevance scores, intended use, and AI Performance tables tell you when results stop being meaningful.
