Indications for Use

The device is intended to aid in locating anatomical structures and for the spatial positioning and orientation of instrument holders or tool guides to be used by neurosurgeons for navigating and guiding compatible surgical instruments in open or percutaneous procedures. The device is indicated for any neurosurgical procedure in which stereotactic neurosurgery may be appropriate and where reference to a rigid anatomical structure, such as the skull, can be identified relative to anatomy images.

Device Story

Geniant Cranial is a navigated neurosurgical positioning robot; supports real-time surgical navigation using pre-operative CT/MR images. System includes console with optical sensor, robot stand with robotic arm, and application software. Surgeon creates/saves surgical routes and 3D anatomical models pre-operatively. During surgery, system tracks surgical tools relative to patient anatomy; updates tool positions on screen; guides surgeon to follow planned trajectory. New SW V3.2 feature allows robot movement while Robot Tool Guide (RTG) or Robot Tool Drive (RTD) is attached. Used in operating rooms by neurosurgeons and surgical staff. Provides real-time visual feedback to assist surgical decision-making and trajectory alignment. Benefits include improved workflow efficiency and precise instrument positioning.

Clinical Evidence

Bench testing only. Performance evaluated using representative worst-case configurations for stereotactic procedures. Accuracy metrics: 3D positional accuracy mean error ≤1.5 mm; trajectory angle accuracy mean error ≤2.0 degrees. Testing included Paired Point Registration, Bone Fiducial Registration, and Tracing Registration methods. All design inputs and performance specifications met.

Technological Characteristics

Hardware platform: console with optical sensor, robot stand with robotic arm, patient positioning module. Sensing: infrared camera and robot arm relative encoders. Connectivity: DICOM import/export via CD/DVD/USB. Software: KGuide V3.2. Sterilization: non-sterile instruments with disposable sterile drapes/covers. Safety standards: IEC 60601-1, IEC 60601-1-2, ISO 10993-1, IEC 62304, ISO 17665-1.

Indications for Use

Legally Marketed Predicate Device(s) Device name: Geniant Cranial (Navigated Neurosurgical Positioning Robot) 510(k) number: K241333 Manufacturer: KOH YOUNG TECHNOLOGY Inc. 5. Device Description Geniant Cranial is a hardware platform that supports real-time surgical navigation using medical patient images. The application software reprocesses the CT or MR images of the patient acquired before surgery. {5} K253856 Page 2 of 8 It displays the contents on the software screen in various fluoroscopy directions (axial, sagittal and coronal). Before surgery, the surgeon can create and save one or more surgical routes to simulate. The surgeon may create and manipulate one or more 3D models of human anatomy before the surgery. During surgery, the system tracks the position of special surgical tools in the patient's anatomy and continuously updates the positions of the surgical tools in these images. The application software can also show you how the actual position and path during surgery relate to the pre-operative plan and guide the surgeon to follow the planned trajectory. The real-time location information obtained through Geniant Cranial can help guide the surgeon's decision and the surgical route. This 510(k) submission addresses a new software feature that enables robot movement with the Robot Tool Guide or Drive (RTG or RTD) attached, providing improved workflow efficiency for the user. All other system functions, intended use, and established safety features remain unchanged. ### 6. Indication for Use Statement The device is intended to aid in locating anatomical structures and for the spatial positioning and orientation of instrument holders or tool guides to be used by neurosurgeons for navigating and guiding compatible surgical instruments in open or percutaneous procedures. The device is indicated for any neurosurgical procedure in which stereotactic neurosurgery may be appropriate and where reference to a rigid anatomical structure, such as the skull, can be identified relative to anatomy images. ### 7. Substantial Equivalence Discussion The following table compares the Geniant Cranial (SW V3.2) to the predicate device with respect to indications for use, principle of operation, technological characteristics, materials, and performance, and forms the basis for the determination of substantial equivalence. #### Special Note on SW V3.2: The subject device, Geniant Cranial (SW V3.2), introduces a new optional software feature allowing robot movement while the Robot Tool Guide (RTG) or Robot Tool Drive (RTD) is attached. This new functionality—developed in direct response to user requests for improved workflow efficiency—was found to introduce a newly identified risk of increased potential for patient collision and tissue injury. These risks were thoroughly analyzed and mitigated by the implementation of new software control measures and detailed user instructions. Due to this risk identification, a new 510(k) is being submitted for SW V3.2 in accordance with FDA guidance. Other updates made since the predicate device relate to usability improvements, reliability, and defect correction and were not found to have a significant impact on device safety or effectiveness. Discussion of the Key Performance (Accuracy) Testing To simulate the clinical procedure as closely as possible, Koh Young considered all parameters that may influence the stereotactic procedure, which include the different devices, accessories, and components of Geniant Cranial, other influencing devices used in the procedure that are validated to be compatible with Geniant Cranial, and other elements of the surgical environment. Under the representative worst-case configuration considering an actual clinical procedure, the Geniant Cranial has demonstrated performance in 3D positional accuracy with a mean error ≤1.5 mm and trajectory angle accuracy with a mean error ≤2.0 degrees. Below are the test results. Statement of Substantial Equivalence Geniant Cranial (V3.2) has the same indications for use as the predicate device. The newly identified risk associated with the additional robot movement feature has been addressed through thorough safety analysis, risk control measures, and software validation. Other minor differences in technological characteristics have been successfully evaluated through appropriate safety and performance testing, and do not raise new questions of safety and effectiveness. Therefore, Geniant Cranial (SW V3.2) is determined to be substantially equivalent to the predicate device.
