## Indications for Use

OSS System Indications: • Painful and disabled joint resulting from avascular necrosis, osteoarthritis, rheumatoid arthritis, or traumatic arthritis. • Correction of varus, valgus, or posttraumatic deformity. • Correction or revision of unsuccessful osteotomy, arthrodesis, or previous joint replacement. • Ligament deficiencies. • Tumor resections. • Treatment of non-unions, femoral neck fracture, and trochanteric fractures of the proximal femur with head involvement, unmanageable using other techniques. * • Revision of previously failed total joint arthroplasty. • Trauma. Zimmer Biomet OSS Reduced size (RS) components offers a variety of component options for treatment in small adults and adolescents (12-21 years) that require proximal femoral, distal femoral, total femur, or proximal tibial replacement as well as, resurfacing components for the proximal tibia and distal femur (Solely indicated for the USA). * Not applicable to Regenerex® Ultra Porous Construct titanium knee augment usage (not licensed in Canada), or any other knee component. Compress System Indications: The Compress Segmental Femoral Replacement System is indicated for: • Correction of revision of unsuccessful osteotomy, arthrodesis, or previous joint replacement. • Tumor resections. • Revision of previously failed total joint arthroplasty. • Trauma. The Compress Segmental Femoral Replacement System components are intended for uncemented use. When components of the OSS System are used with Zimmer Biomet Compress Segmental Femoral Replacement System, the user should refer to the package insert of the Zimmer Biomet Compress Segmental Femoral Replacement System components for information.

## Regulatory Identification

A hip joint metal/polymer semi-constrained cemented prosthesis is a device intended to be implanted to replace a hip joint. The device limits translation and rotation in one or more planes via the geometry of its articulating surfaces. It has no linkage across-the-joint. This generic type of device includes prostheses that have a femoral component made of alloys, such as cobalt-chromium-molybdenum, and an acetabular resurfacing component made of ultra-high molecular weight polyethylene and is limited to those prostheses intended for use with bone cement (§ 888.3027).

We have reviewed your section 510(k) premarket notification of intent to market the device referenced above and have determined the device is substantially equivalent (for the indications for use stated in the enclosure) to legally marketed predicate devices marketed in interstate commerce prior to May 28, 1976, the enactment date of the Medical Device Amendments, or to devices that have been reclassified in accordance with the provisions of the Federal Food, Drug, and Cosmetic Act (the Act) that do not require approval of a premarket approval application (PMA). You may, therefore, market the device, subject to the general controls provisions of the Act. Although this letter refers to your product as a device, please be aware that some cleared products may instead be combination products. The 510(k) Premarket Notification Database available at https://www.accessdata.fda.gov/scripts/cdrh/cfdocs/cfpmn/pmn.cfm identifies combination product submissions. The OSS™ Orthopedic Salvage System offers a variety of component options for treatment of patients that require proximal femoral, distal femoral, total femur, or proximal tibial replacements, as well as, resurfacing components for the proximal tibia and distal femur. The OSS System offers the surgeon treatments for patients presenting bone loss and deformity associated with bone tumors resection, trauma, and difficult revision arthroplasty. Zimmer Biomet OSS Modular Arthrodesis System and specialty components hip soft tissue claws are also used with the Zimmer Biomet OSS Orthopedic Salvage System.
