Indications for Use

Venti-Trach Tracheostomy Tube is intended for the airway management of tracheostomized adult patients. The device is operated by health professionals in medical institutions for single patient use. It may be continuous use by replacement and each one should be used within 29 days.

Device Story

Respiratory device inserted into trachea via stoma; provides artificial airway for ventilation and oxygenation; bypasses upper airway obstructions. Consists of cannula, 15mm connector, neck plate, pilot balloon, and optional cuff. Cuff seals trachea for positive pressure ventilation; fenestration (if present) allows airflow for speech. Operated by health professionals in critical/acute care or long-term facilities. Secured via neck strap and locking mechanisms. Introducer assists insertion. Output is a patent airway for mechanical ventilation or spontaneous breathing. Benefits include restored breathing pathway for patients with airway injury or obstruction.

Clinical Evidence

Bench testing only. Verification and validation performed per ISO 5366:2016, ISO 5356-1:2015, ISO 10993-1:2018, and ASTM F1980. Biocompatibility evaluation (RDE-23090001) conducted for materials.

Technological Characteristics

Regulatory Classification

Identification

A tracheostomy tube and tube cuff is a device intended to be placed into a surgical opening of the trachea to facilitate ventilation to the lungs. The cuff may be a separate or integral part of the tracheostomy tube and is, when inflated, intended to establish a seal between the tracheal wall and the tracheostomy tube. The cuff is used to prevent the patient's aspiration of substances, such as blood or vomit, or to provide a means for positive-pressure ventilation of the patient. This device is made of either stainless steel or plastic.

For devices of the same size specification, the total tube length differs by less than 10 mm, the outer diameter by less than 0.7 mm, and the cuff diameter by less than 2 mm. These geometric differences have been verified through applicable tests in accordance with ISO 5366:2016 (see Design Control Activities Summary), and are disclosed in the Instructions for Use (IFU) and product labeling. # **Analysis 2:** The subject device differs from the predicate device by a material change in the neck plate, from TPR to PVC. No new biocompatibility testing is needed because the same PVC material is already used in the predicate device's tube, which has direct airway contact. Since the subject device's neck plate only touches intact skin, the risk of a new biological response is considered lower than what has already been proven safe, thus justifying the change without additional testing. Please refer to Biocompatibility Evaluation (RDE-23090001) for the detail justification of the biological risk. # **Analysis 3:** The subject device does not include a disconnect wedge. According to the following, the absence of a wedge does not impact device safety or performance. (1) The neck plate material was changed from TPR to PVC, providing higher rigidity and stability so that removal does not require auxiliary tools. (2) The 15 mm connector conforms with ISO 5366 and ISO 5356-1, allowing safe removal by standard handling. (3) The wedge is not a standard requirement. Some marketed models, such as the marketed device K173384 (IFU #10001029-004), do not include this accessory. # **Conclusion** According to the discussion above, the indications for use of subject device, Venti-Trach Tracheostomy Tube is same as predicate device, Rota-Trach Disposable Standard Tracheostomy Tube, and the other differences in non-significant characteristics are clarified, the substantial equivalent is claimed. Since the differences of the devices do not raise different questions of safety and effectiveness, Venti-Trach Tracheostomy Tube is as safe and effective as legally marketed Rota-Trach Disposable Standard Tracheostomy Tube. # **7. Design Control Activities** The risks arisen from the design modifications activities have been identified and evaluated are controlled through a failure modes and effects analysis (FMEA) that described in FDA-recognized standard, ISO 14971:2019. The safety and effectiveness of Venti-Trach Tracheostomy Tube are also verified and validated according to device-specific, FDA-recognized standard, ISO 5366:2016, ISO 5356-1:2015, ISO 10993-1:2018 and ASTM F1980. 5-6 {10} ## **8. Conclusion** Based on the information provided in this premarket notification, the same indications for use with small modifications, Venti-Trach Tracheostomy Tube raises no new questions of safety and effectiveness and subject device is substantially equivalent to predicate device in terms of safety, effectiveness, and performance. 5-7
