Device Story
Medley Gold series is a customized mandibular repositioning device (MRD) used to treat snoring and mild-to-moderate obstructive sleep apnea. The device consists of custom-fitted upper and lower dental trays fabricated by a dentist. Three interchangeable attachment options—elastomeric bands, links, or rods—are used to advance the mandible relative to the maxilla, thereby enlarging the airway during sleep. The device is intended for use in home, dental office, or sleep laboratory settings. By holding the lower jaw in a forward position, the device helps maintain airway patency. It allows for lateral and vertical jaw movement. The dentist selects the attachment method based on patient preference and comfort, and adjusts the degree of protrusion. The device is a single-patient, multi-use appliance.
Clinical Evidence
No clinical data. Substantial equivalence is based on bench testing of materials and mechanical components, and comparison of design, indications, and principle of operation to legally marketed predicate devices.
Technological Characteristics
Customized oral appliance consisting of upper and lower trays fabricated from Durasoft, Splint Biocryl, and Biocryl X glue. Mechanical advancement components include elastomeric bands, links, or rods with associated nuts, screws, and mounting frames. Protrusion is adjustable in 1 mm increments (up to 8 mm for bands/rods). The device allows for lateral and vertical jaw movement. Cleaning is performed by rinsing with water. No electronic or software components.
Indications for Use
Indicated for adult patients 18 years or older with night-time snoring and mild to moderate obstructive sleep apnea. Contraindicated in patients with central sleep apnea, severe respiratory disorders, loose teeth, or advanced periodontal disease.

The devices are designed to increase the patency of the airway and to decrease air turbulence and airway obstruction. The classification includes palatal lifting devices, tongue retaining devices, and mandibular repositioning devices.

#### Device Description
The Medley Gold series oral appliance design concept is based upon the use of a standard set of upper and lower trays that have been customized by a dentist that then may have one of three (3) options attached to the trays to act as a Mandibular Repositioning Device (MRD).
The rationale for have a single set of customized trays that may have different MRD methods attached is that some patients have a personal preference and some configurations are more comfortable for them. Therefore rather than having to make another set of custom travs, the dentist may use the same trays and just change the method of MRD, i.e. bands, links, or rods, to the same set of trays.
The principle of advancing a lower tray so that it advances the mandible for the treatment of snoring and / or obstructive sleep apnea is well known and there a number of predicate devices.
The proposed Medley Gold device has three (3) methods of advancing the lower tray based upon the identical designs of 3 already cleared predicate designs.

#### Non-clinical Testing -
FDA has suggested that all intra oral devices will have to demonstrate that the physical properties of the materials used to fabricate the devices are equivalent to that of the predicate device. For the proposed Medley Gold customized oral appliance, all the materials for the customized tray are being used as directed by the manufacturer and are unaltered.
The materials are currently used to make customized trays which are identified predicates that also use customized trays as their base design then adds components which are used to advance the lower jaw and hold it in place while the patient sleeps.
These components, bands, links or rods, are similar to the predicates and we performed specific tests to validate these components and the method of attachment to the trays.
However the list of tests, ultimate flexural strength, ultimate flexural modulus, water sorption, water solubility, fracture toughness with modified bending test, relate to the tray materials, all of which are standardized dental materials that are FDA listed.
For those parts of the Medley Gold which are not tray materials, we performed testing to demonstrate that the support components perform as intended

3. Judging fuzzy relevance
Fuzzy results trail off in relevance as you scroll. Use three signals to decide how far down to go: the fuzzy badge explanations, the intended use column, and whether your target output (e.g., Cobb angle) still appears in the AI Performance sub-table. Once it stops appearing, you're past the relevant zone. A top hit with a low score (~0.4) and a stretched explanation is a hint the closest predicates are far away — the project may be headed for De Novo. Note the fuzzy search is a pattern match: it doesn't handle negation ("not") well, and hardware devices can appear — filter by SaMD/AI ML to cut them.

4. Device detail page: chat and citations

5. Side-by-side comparison
Select multiple rows in the results table (aim for under ~10), then open the PDF Viewer tab. Ask one question — it goes to all selected devices in parallel, each with citations. This is the fastest way to compare and contrast devices: training data, PCCP scope, how they handled adding new scanners, and so on.

6. Collections

7. Product codes and the regulations tree
Click a product code in the results to jump to it in the regulations tree — identification text, sibling product codes, and devices you can open in a PDF viewer on the right. Click a regulation number to see its identification, special controls, and related product codes. You can also search by product code or regulation number at the top of the tree. Always read the special controls if any exist for your device — it broadens your search and sharpens pre-kickoff research.

8. Chart view
Click Show Chart and segment by regulation number (or product code) to see which regulations dominate your result set. Clicking a regulation takes you into the regulations tree. Great for spotting that most matches are, say, hardware laparoscopic devices — a cue to go back and filter.

9. The predicate graph
Open the Predicates tab for a family-tree view of predicate relationships. Click a node to trace its parents and children; selections from search carry over pre-selected. 10. Embeddings: the galaxy map
The Embeddings tab plots every matching document in a 2-D "galaxy map" where semantically similar devices cluster together. Hover or click clusters to explore, and let AI label the clusters for you. Embeddings beat product codes for grouping: two devices can carry different product codes (LLZ vs. QIH) yet do the same thing — the embedding captures the meaning of the intended use and device story. This is also exactly how retrieval-augmented generation (RAG) works under the hood, and it makes a great visual on client calls.
